Each of our clinicians are trained in how to conduct a gender affirming assessment for the purpose of making referrals for surgery. Our fee for this service is pay what you can from $0-clinician full fee. In the majority of situations, we are able to complete the assessment in one session and provide a letter within 48 business hours. Our team uses the informed consent model. This means we are assessing for two things, first, do you meet the diagnostic criteria for Gender Dysphoria and second, do you have the ability to engage in informed consent regarding your medical care. When reaching out to our team for an assment/referral letter please be prepred to provide us with the following information:

-Must be a located in GA or FL at the time of the assessment, please identify which state when you reach out

-Certificate of coverage for gender affirming care from your insurance company including any requirements as to what must be included in the letter and who must conduct the assessment (PhD, MD, LCSW, LPC, etc)

-Name and CPT Codes for the procedure you are specifically seeking (can be obtained from your surgeon)

-Information or documentation your surgeon/physician has provided that outline any requirements they may have

PLEASE NOTE WE DO NOT PROVIDE ASSESSMENT AND REFERRAL LETTERS FOR ADULT GENDER AFFIRMING HORMONE THERAPY. This service does not require a referral letter based on WPATH SOC7 or SOC8 recommendations. Supervision for non-Modern Path team members is available for developing Sex Therapists who are centering the experiences of queer and trans clients by Nick Marzo. Nick uses a developmental supervision model that helps you set professional goals and measure outcomes based on your skill development.
