Target Population and Workflow
Definitions:
Client: An established client of an Approved Provider receiving individual therapy services. An established client has received a minimum of eight sessions prior to applying for funds.
Advocate: An Approved Provider who recognizes a need and supports the application process.
Program Manager: Designated team member who implements and manages the Modern Path Access Fund.
Direct Financial Assistance: $1800 stipend to supplement sessions fees for a period of up to six months wherein the session fee does not exceed $150 during the time period the individual is accessing the fund.
MPAF Direct Financial Assistance Program
Clients in need of Direct Financial Assistance will be identified by their Advocate (an Approved Provider of the MPAF). The client and advocate will apply for Direct Financial Assistance by completing the application available on the Modern Path Access Fund website. A decision will be made by the Program Manager within 30 days, allowing adequate time to determine eligibility and availability of funds.
Process:
Client and Advocate identify a need
Client completes application with support of Advocate
The Program Manager reviews the form and supporting documentation.
The advocate and Program Manager will collaborate to determine the client's eligibility based on the established criteria and prioritize accordingly.
If further clarification or documentation is needed, the Advocate and Program Manager will engage in a collaborative phone call.
Once eligibility is confirmed, then the Program Manager will inform the Advocate and client of award status and provide the MPAF documentation to be reviewed and signed by both the client and the Advocate.
Eligibility for access to MPAF is defined as
Need:The client has been assessed to benefit from outpatient psychotherapy services with a therapist trained in the identified specialty areas;
The loss of services due to financial hardship would create a greater risk of emotional or cognitive impairment AND;
The financial hardship is assessed to be short term in nature lasting no more than six months.
Client meets the target population as defined by:

Applicant is a member of a historically oppressed gender experience
Applicant is impacted by a physical disability
Applicant is a pregnant birthing parent
Applicant is in an at-risk or insecure living situation
Applicant is a migrant
Applicant is an adoptee
Applicant is an older adult (55+)
Applicant is a transitional age adult (18-24)
Decision Work Flow:
Minimum of 3 items met, 1 must be from Category A

Applicant is a transitional age adult (18-24)
Award Specifications and Limitations
Individual Therapy access.
Once a client is determined to be eligible for the MPAF, the client will receive a maximum of $1800 as unused credit on their Modern Path Client Account for use over a period of 180 days. Clinicians agree to provide the service at rate not to exceed $150. If, during this time, financial status changes and the client is able to pay a higher portion or the full fee for sessions, the unused funds will be reallocated to the MPAF for redistribution.
Medical Record and Administrativ Documentation Requirements
Upon receiving approval for the fund the clinician agrees to:
Complete a GAD-7, RSES, and PHQ-9 and submit at day 1, day 90 and day 180 of the award period
Maintain an up to date and accurate treatment plan including an objective related to financial security
Complete session notes for each session provided
Submit an invoice and required documentation to MPAF and await reimbursement
